Where to park:

In front of the office on High Street/Route 19.

Turn into the alleyway between 4th and 5th Streets, there is parking available next to the building.

What your should bring:

Bring your ID and your insurance card with you. We can bill your insurance on your behalf.

Bring in X-ray reports, images, medical documents, prescription list (your pharmacist can print off a list of current medications) and any other relevant documents.
